I based this report design on Dr. Joseph Burrascano’s symptom checklist from his publication titled “DIAGNOSTIC HINTS AND TREATMENT GUIDELINES FOR LYME AND OTHER TICK BORNE ILLNESSES”. The article can be found at http://www.ilads.org/burrascano_0905.html. Dr. Burrascano is a very well respected Lyme Literate MD (LLMD) and the former head of ILADS. I would like to thank him for all of his amazing work for the Lyme disease community.

Instructions:

You will be prompted to enter your name, date, and email address. Please make sure you use the same email address each time you take this survey. This is the email address that your report will be sent to. Enter the date that you are entering the symptoms for. This is a daily symptom tracker, so the date is used to keep your report in order.

You MUST email me when you want your report sent to you. I will send it out as soon as possible. This project is still in it’s infancy and I have to import the results manually and send the report to you. I’m trying to automate this process, but I haven’t quite figured it out yet.

    There is already a website that does this and more; www.lymelog.com. Why reinvent the wheel?

    This is TOTALLY FALSE. LymeLog.com has been around for almost two years and is completely free. It gives you an instant graph with the click of a button, no need to have anything emailed to you. It has many functions.

    The person who developed it has always provided it for free. Please do not make false claims on the internet.

    I’m sorry Tracy, but I had not heard of this other site. I developed this for my own use and decided that it would be helpful to others with Lyme. I’m not trying to deceive anyone at all. I’m just trying to help. I hope you’re doing well in your treatment. Have a happy and healthy new year.

    I checked www.lymelog.com site. It’s VERY different; YOU have to write your symptoms yourself.
    Off course, it’s better when you can just push the button and print the report. But I like this site more. I found 95% of my symptoms in the list, it’s much easy for me then write each of them myself.
